
Policies & Procedures
Cancellation Policy: A fee of $45 will be billed to the client or patient for cancellations that do not provide 24-hour notice prior to scheduled appointment. Please note: Insurance companies are not responsible for and will not pay these fees.
No Show Policy: The standard fee for scheduled service will be billed to the client or patient in full for missed appointments with no prior notice. Please note that insurance companies are not responsible and will not pay these fees.
Refund Policy: No refunds are available for Gift Certificates or Pre Pays, however they are transferable within a reasonable amount of time (one year from date of issuance).
Pricing: All pricing is location specific and based on payment received at or prior to time of service.
Insurance: Patient is responsible for all charges whether or not paid by insurer and will agree not to withhold or delay payment if insurance company denies charges for any reason. Co-Pays are due at time of service.
Session Parameters: All session's begin at the appointed time and end at the end of appointed time. Please come 5 minutes early to your initial appointment to allow time for intake paperwork. Initial appointment may require evaluation, which is also included within the appointed time. This is essential and allows the therapist to become familiar with your needs & preferences in order to proceed with the best plan of care. Time savers: Have your payment or co-pay ready (cash & checks are quicker) and be prepared to ask questions or express your needs so we can start the important work.
