For cash clients with no ongoing or current health conditions or cautions around your therapy, you only need to fill out 4 forms: Client Intake, S.O.A.P., HIPPA & Policies. 

Client Intake Form

If you are a first-time client, please print this form, complete it and bring it along to our first session.

Insurance Intake Form

Information needed prior to receiving medically necessary Massage Therapy

S.O.A.P. - Visual Complaint Intake Form

Circle areas of concern and describe complaints

HIPPA Release Form

Authorization to release medical information to selected individuals or groups

Policies & Procedures (Signature required)

This form aids our clients / patients the ability to understand how we conduct our business, prior to engaging. It allows you to read, ask questions, understand and agree to uphold the policies, therein. Your signature is a legal statement. Please be sure you read this document carefully prior to signing.

MVA / PIP FORM: Auto Accident Claim Information

Please make sure you have an open and viable PIP claim Answer all questions & sign prior to your initial appointment.

Medical Insurance Plan & Updates

It is your responsibility to be aware of changes to your insurance plan and to make your practitioner aware in a timely fashion. Please take advantage of this form and share it with your therapist ASAP, when changes to your plan occur.
